Providing Care at Home to the Frail Elderly

The Multipurpose Senior Service Program (MSSP) is a voluntary care management service. The program provides care management, which includes information and assistance about appropriate community resources to meet needs, and monitoring of the client's on-going care and health status. A care manager and a public health nurse visit the client in his or her home, meet with family members as needed, and work with the individual to create a plan of care so they can remain at home.

Who Qualifies for MSSP?

MSSP clients must be residents of El Dorado County and:

  • Age 65 or older
  • Receive Medi-Cal under an appropriate aid code
  • At risk of being placed in a skilled nursing facility or intermediate care facility due to frail health
  • Appropriate for care management services
  • Voluntarily accept MSSP services

The MSSP enrollment plan begins with a comprehensive lifestyle evaluation. After assessing the client's needs, the care team works closely with the client to determine what they need to remain safely at home. The team provides information and assistance to the client and family and helps identify and coordinate a broad range of services which may include:

  • In-home support services
  • Counseling
  • Respite care
  • Adult Day Care
  • Transportation
  • Emergency response services
  • Supplies & equipment
