  • Autism Registration Form -  The information provided on this form will be utilized to identify your residence and the individual with autism.

  • Civilian Complaint Procedure and Form - Personnel complaints are defined as any allegation of misconduct or improper job performance against any department employee.

  • Clearance Letter Request - A clearance letter is a criminal history check on you for any arrest (we release only El Dorado County Sheriff’s Office arrest information, no other agencies arrest information will be provided). Bring it with you to your appointment at the Sheriff’s office. 

  • Historical Information Request - California Assembly Bill 1424 (2001), now a law, requires all individuals making decisions about involuntary treatment to consider information supplied by family members and other interested parties. 

  • Local CORI Request - Subjects may request a copy of their local criminal history (El Dorado Sheriff’s arrests only will be provided). You will be required to come in person with the completed form, show a valid ID and pay the fee before the request will be processed.

  • Probable Cause Declaration (PCD Form) -  The declaration in support of the warrant of probable cause for arrest.
  • Trespass Enforcement Authorization Form  -  Allows the property owner to give trespass prosecution authority to another individual.
  • CCW Gun Class & Safety Verification Form  -  This form should be completed in conjunction with your new or renewal CCW application.
